While there are no formal CALO memberships or membership dues at this time, we value the ongoing camaraderie and service provided by our volunteers.

New volunteers wishing to participate in outreach flights will need to follow the links below to complete the one-time Volunteer Registration Form and General Liability Release Form. They will also need to complete the Clinic Registration Form for each scheduled clinic. Pilots will need to complete the Pilot & Aircraft Information Form and the Pilot Affirmation & Waiver Form. Health professionals are required to complete the Professional Resume Form. Returning volunteers, already registered with CALO, will simply need to complete and submit the Clinic Registration Form at least 60 days before a scheduled clinic flight. These documents can be completed, signed electronically, and submitted through this website.

Everyone crossing the border will need a current Passport. Please be certain that you have a current Passport, or that you can obtain one (see your local US Post Office for a passport application appointment; processing usually takes about 4-6 weeks) prior to the trip.

Volunteers will need to make their hotel reservations at the La Villa de San Quintin Hotel on their website http://www.hotellavilla.biz/sanquintin prior to the clinic weekend to guarantee rooms. CALO usually reserves a block of rooms that the hotel will hold available until two weeks before the clinic. After that time, only rooms reserved by individuals will be retained for our team. In the unlikely event that La Villa de San Quintin Hotel becomes overbooked, we suggest making reservations at the neighboring Maria Celeste Hotel at O11-52 Country Code + 616-165-3999 (request English speaker if needed). As the clinic date approaches, CALO will send emails to the team indicating names and contact information for volunteers registered for the trip. For those volunteers wishing to share a hotel room, CALO will indicate which volunteers need roommates.

Volunteers will be contacted by a Pilot Coordinator to learn specifics about the departure location and time for their assigned aircraft.

You will need the Acrobat Reader to open some Forms. It can be downloaded from the Adobe site: http://get.adobe.com/reader/
